We provide a community‑based service for people aged 65 and over with acute mental health needs, and for people under 65 with a diagnosis of dementia who are living in a care home.
We also support care homes to manage behaviours that can be challenging.
We work closely with care home staff to help you remain where you are, to avoid unnecessary moves to other care settings or hospital.
Our team is made up of a team manager, registered mental health nurses with experience in dementia care, a support worker who works alongside the nurses, and a team administrator.
If you work in a care home, we can provide support through therapeutic approaches, including life story work and advice on developing care plans.
We carry out comprehensive mental health assessments in partnership with you, your families and carers. Where appropriate, you can be referred to other services, such as psychiatry, memory assessment services and adult social care.
