Community learning disability team, Bury
We help people with learning disabilities, who are registered with a Bury GP.
Our team is made up of adult learning disability nurses, occupational therapists, speech and language therapists and psychologists.
We provide specialist assessments and intervention plans for individuals with a diagnosed learning disability. These focus on health inequalities, positive behaviour support, occupational therapy assessment, communication, and psychological needs.
We work with your family and other agencies to achieve positive outcomes for you.
We accept referrals from adults with a diagnosed learning disability, carers, social workers, GPs and other health professionals.
All referrals will be checked to see if you are eligible for support from us.
