Adult Learning Disability Team, Bury

Our team is made up of learning disability nurses, occupational therapists, speech and language therapists and psychologists. We work together to provide joined up care to adults aged 18 years and above.

We support individuals who have a mild, moderate or severe learning disability who are registered with a Bury GP.

We aim to provide a high quality services that meet's people needs and to make sure that each individual can live a valued life in the community.

This is underpinned by the key principles of inclusion, rights, choice and independence.

The core aim of the service is to deliver a needs led, highly specialist assessment and intervention plan focusing on health inequalities, behaviour, risk, sensory, communication and psychological needs to individuals with complex and challenging health needs.

We will work in partnership with other agencies and / or families to achieve positive outcomes for individuals in the home, community and other relevant settings. We accept referrals from service users, carers, social workers, GP's and other health professionals. All referrals will go through a screening process for eligibility to access the service.

In previous years the service was voted by colleagues as winners of the Principles of Care Awards for their work with people with Down's Syndrome and dementia.
