Take control of your appointments and care

We're introducing a new online patient portal to help you manage your appointments and care more easily.

Starting soon, we will:

  • Send you appointment reminders either through the NHS App, text message or email (based on how you like to hear from us) for your outpatient and community appointments
  • Send you digital forms and assessments related to your care. You will be notified when you have a form to complete either through the NHS App, text message or email.

Through the patient portal, you will be able to:

  • See your upcoming appointments on your phone, tablet or computer
  • Get reminders before your appointments
  • Complete forms online before your visit
  • Update your contact details easily

We are piloting appointment reminders across our outpatient and community services in Bury from November 2025 before we roll-out to other services. We will be rolling out digital forms and assessments early 2026.

1. Receiving your link

When you have an appointment, you’ll receive a message by SMS or email with a link to log in to the patient portal.

2. Signing In

To protect your privacy, you must sign in before viewing any personal information. The first time you sign in, you’ll be asked to agree to our privacy policy.

You can choose one of two ways to sign in:

  • If you have an NHS App account, click “Continue with NHS login” and follow the steps.
  • If you don’t have an NHS login, you can sign in using your last name, date of birth, and postcode. Make sure these details match what we have on record.

Trouble Logging In?

If you can’t log in, please check with your care team that we have your correct surname, date of birth, and postcode. If your details have recently changed, it may take up to 48 hours for updates to take effect.

3. Security Step

After entering your details, you’ll receive a one-time code (OTC) by text message or phone call. Enter this code to finish signing in. If you enter the wrong code three times, you’ll need to start again.

Once you’ve logged into the patient portal, you’ll find all your upcoming appointments listed under the appointments section.

How to view your appointment details:

  • Select an appointment from the list to see more information.
  • On the appointment screen, you can:
    • View the date, time, and location of your appointment.
    • Get directions to your appointment.
    • See additional clinic information, such as contact details.

Need to make changes or cancel?

  • If you need to cancel or change your appointment, you’ll find contact information for your clinic on the appointment details page.
  • You can also add your appointment to your calendar with one click.

You can choose how you’d like to receive appointment notifications and reminders by updating your contact preferences in the patient portal.

How to update your contact details:

  • Click on Settings at the top right of the patient portal screen.
  • Select which phone numbers or email addresses you want to use for communications by switching the toggle buttons on or off.
  • You can add extra email addresses or mobile numbers if you’d like family members or carers to also receive updates about your appointments.

Important:changes made in the patient portal will only affect messages sent from the portal itself. To make sure we can always reach you, please also let your care team know directly if you change your contact details.

f you need help with accessibility or language, select Accessibility and Language Options at the bottom of the patient portal page to open the Recite Me toolbar.
