This can include, but isn’t limited to:
- Psychosis
- Bipolar disorder
- ‘Personality disorder’ diagnosis (also know as complex, emotional and relational needs)
- Severe depression
Referrals are via any professional – for example, GPs, health and social care, education, police and the probation service.
About the service
The service is run by Pennine Care, Bury Council, BIG in Mental Health, The Creative Living Centre and Early Break.
It brings together experts from mental health, social care, primary care (GPs), housing, employment, substance misuse, people with lived experience of mental health and more.
As part of the biggest-ever transformation of community mental health services, this is the first time so many different services have worked together as a true partnership. And people with lived experience of mental health issues have shaped every step of our development.
You can easily and quickly be referred to the right professional or service for your needs.
You might receive support from one or more professionals at the same time.
Sharing your information
To make sure we can effectively support you, we’ll need to share key pieces of information about you with all professionals involved in your care. This will include personal details (e.g. name, date of birth, address, NHS number etc), details about your condition, treatment and care and other vital information.
Your information will always be held and shared in a safe and secure way in accordance with UK data protection laws.
Read more about how we use and safely store your information.
How to get help
The service is for people aged 18+, registered with a GP in Bury, who are experiencing mental health issues .
Your GP can refer you to the Bury living well service and someone will be in touch.
Consultant access service for GPs
We've developed a consultant access service to provide GPs with quick access to consultant psychiatrist advice and rapid feedback for Bury patients.
Patients not already receiving support from secondary care mental health services can be referred to the consultant access service by their GP.
A consultant will respond to the GP's referral within three working days. They can provide medication and diagnostic advice, clinical management and signposting.
Consultant access ensures patients receive efficient and high quality care and helps GPs feel better supported to care for patients with mental health issues.
GPs can refer to the consultant access service using the existing single point of entry form and specifying ‘consultant access’ (the form is available on the EMIS system).
