Children’s community learning disability team, Bury
We help children with learning disabilities who have a GP registered in Bury.
We also help families, carers, wider services and professionals, including schools, social care and health care.
Our team is made up of learning disability nurses.
We offer assessments for health, behaviour, and sleep.
We can help with:
- Advice for families and carers
- Individual behaviour support plans
- Sleep assessments and plans
- Health assessments
- Identifying reasonable adjustments you need to access wider health care
- Hospital passports
We accept referrals from young people, families, carers, social workers, GPs and other health professionals.
All referrals will be be checked to see if you are eligible for support from us.
