We are a community-based specialist mental health team for people over the age of 65 years with complex needs.
We offer a range of assessments and interventions to help us understand your needs and provide treatment with minimal disruption.
Our aim is to bridge the gap between hospital and community services through safe and effective alternatives to hospital admission. We can also support you to be discharged from a psychiatric ward, as early as possible.
We do this by:
- Acting as gatekeeper for all admission requests (during 9am - 5pm) and linking with the inpatient service to support early/timely discharge, where appropriate.
- Providing a swift response if you are experiencing a mental health crisis.
- Short-term support and treatment in your home environment.
We are a team of different health and care professionals, comprising of a psychiatrist, registered mental health nurses, an occupational therapist, and healthcare support workers.
Visiting information
We offer home appointments and dependant on need; this can be more than once a day.
There may be times visits are conducted in pairs but you will be informed of visit schedules ahead of time.
