Referral to Bury Home Treatment is an alternative to hospital admission. Staff provide monitoring, treatment and support to the individual in their own home.
Care includes monitoring the effects of medication, risk assessment and mental state examination on each contact. Should risk escalate in any of the four domains or mental state deteriorate to the extent that an admission is required this can be facilitated by the team, supported discharge is implemented wherever possible to reduce the length of stay.
Mental health professionals can refer to the service following a completed mental health needs and risk assessment.
Referral should be made to the gatekeeper.
