We provide specialist mental health assessments and support to adults over the age of 65 who are living in the community. We can also support you if you're aged under 65 and have a confirmed diagnosis of dementia.

If you're experiencing mental health problems and the professional working with you feels you would benefit from specialist input, they can refer you to our team.

We initially aim to work with you for up to six weeks. If ongoing support is required after this, we would discuss the options with you and your carers, as well as the relevant services involved in your care.

Our service is non urgent - any urgent referrals should be sent via the access and crisis team.

We accept referrals from:

  • Social workers/support planners
  • Community matrons
  • Community mental health teams
  • Rapid response teams
  • Discharge to assess teams
  • The staying well team
  • Parkinson’s nurses.

We don't accept self-referrals or GP referrals.
