The Tameside older people's community mental health team is now known as the Tameside older people's specialist mental health team. We are part of the Tameside and Glossop neighbourhood mental health service.
Our service is for people over 65 with moderate or severe and/or enduring mental health needs.
We provide you with a specialist mental health assessment, treatment plans and interventions.
We can also provide support if you're under 65 and have a confirmed dementia diagnosis. We regularly review and update the treatment plans with you until you’re ready to be discharged.
Our team is made up of mental health nurses, qualified assistant practitioners, support workers, occupational therapists, physiotherapists, clinical psychologists, administrators, and a team manager.
We work closely with the Tameside home intervention team - older people :: Pennine Care NHS Foundation Trust and the older age consultant psychiatrists.
We don’t commission social care packages for you or the carers who support you, but we can make referrals to any services relevant to your care.
Referrals are made via the single point of entry, who screen all referrals and direct them to the most appropriate service for your needs.
