In September 2026 we launched a new and improved service, which brings together assessment, treatment and support together in one joined up service.
We're available 24 hours a day, seven days a week, for adults up to the age of 65 in Stockport. Details of how to get help from us are provided below.
We can support you by:
- Responding quickly when you need urgent help in a crisis
- Providing intensive support at home or in the community to help you stay out of hospital wherever possible.
- Supporting you through your mental health crisis and helping you get the right community-based support once the crisis has passed.
- If you’ve been in hospital, supporting you to return home sooner and continue your treatment and care there.
- Working closely with your GP and other professionals and services, so all your care and support is joined up.
Improvements we've made
Thanks to £4m of extra funding, we've made lots of improvements to our service.
We worked with patients, carers and people with lived experience at every stage of development, to make sure the improvements meet people's needs. Through this work we achieved the "Being Shaped by Lived Experience" award from Pennine Care’s Patient Experience and Engagement Reference (PEER) group.
Some of our improvements include:
- More staff working in our team from different professional backgrounds, including nurses, social workers, occupational therapist, and a specialist doctor
- Medication support and advice from our dedicated pharmacists
- We'll be building on our skills and knowledge through additional training, helping us offer a wider range of support.
- We can now accept referrals from lots of different professionals, and we've improved our referral process - meaning we can offer help in a crisis more quickly and easily.
-
Improved partnership working with other professionals and services, including GPs and local community groups, to make sure support is joined up.
How to get help
Lots of different professionals can now refer you to our service for crisis support. Details are provided in our referral guide.
- Pennine Care professionals - including nurses, social care, occupational health etc.
- Approved mental health professionals (AMHP).
- GPs and other healthcare professionals.
- Fire service (police and ambulance colleagues should contact the mental health urgent triage team).
- Non-mental health professionals (e.g. adult social care, housing, substance misuse).
- Voluntary, community, faith and social enterprise partners (NHS Greater Manchester contracted partners only).
- Physical health hospital and urgent care professionals
- Physical health community professionals (e.g. district nurses, health visitors etc).
If you're a patient on one of our wards, your care team can work with us to make sure you return home as soon as possible.
Unfortunately, we can't accept self-referrals, or referrals from carers or family members.
Support for carers
As a carer, you will be offered the opportunity and time to discuss your concerns with a member of our team. You can always contact us and provide information that you feel is important. The information that you provide is very valuable to us and helps inform our assessment and treatment.
If the person you care for gives us permission to share information, then we can share all aspects of treatment and care.
However, if they don't give permission to share information, we are bound by laws of confidentiality. We will then only be able to share generalised information about our team, mental health illness and treatment in general.
At times, someone may give us permission to share only specific information that is important to their care. If at any times we assess that there is an immediate risk to them, then we may in these instances break confidentiality.
We always encourage people to give permission to share information with you as carers, as we value the important role that you play in their care. But sometimes people wish to withhold information.
We will always ensure you have the necessary information of what to do in a crisis.
We know you play an essential role in helping your loved ones to stay well or get better, and so we are committed to the role of carers and follow the six key standards of the Triangle of Care.
Helpful contacts
As a carer you have the right to an assessment of your needs and a care plan. We will offer you a referral to Mind who will undertake this on our behalf.
