You can write your statement in Word so you can easily track word count and paste the text back into the submission form.
Please provide the following information in your statement:
- Please tell us a bit about your nominee – for example what team, service, project do they lead, how long have they been in this role, and any other relevant background information.
- Describe how they go over and above and inspire others with their communication style, vision, support for others, ability to drive change, or anything else that stands out.
- What impact have their contributions had - please include supporting data, statistics, quotes, or stories from patients, families, colleagues, partners and others (anonymous is ok)
- Please consider how your nominee demonstrates:
- Our values - kindness, fairness, ingenuity, determination
- Our ambitions – outstanding care, great place to work and listen to improve
