Your supporting statement should be a minimum of 200 words and a maximum of 500 words.
You can write your statement in Word so you can easily track word count and paste the text back into the submission form.
Please provide the following information in your statement:
- Please tell us about your nominee – for example their key responsibilities, how long they’ve worked here, their career journey and any other relevant background information.
- Please provide some examples of how they consistently stand out - think about how they work with and support others, the difference they make to patients or colleagues, their achievements, and anything else that stands out.
- What impact have their contributions had - please include supporting data, statistics, quotes or stories from patients, families, colleagues, partners and others (anonymous is ok).
- Please consider how your nominee demonstrates:
- Our values - kindness, fairness, ingenuity, determination.
- Our ambitions – outstanding care, great place to work and listen to improve.
