Inclusion champion criteria 2025

Your supporting statement should be a minimum of 200 words and a maximum of 500 words. 

You can write your statement in Word so you can easily track word count and paste the text back into the submission form.

You should provide the following information, as the judges will use this to score your submission 

  • Please tell us a bit about your nominee and their key responsibilities
  • Describe how they actively champion equality, diversity, and inclusion. How do they go above and beyond?
  • What impact have their contributions had – please include supporting data/statistics, or quotes/stories from patients, families, colleagues, partners and/ or others (anonymous is ok)
  • ​​​Please explain how your nominee demonstrates at least one of our values and why:
    • ​​​​​​​​​​​​​​Kindness: We believe that care and compassion underpin everything.
    • Fairness: We work together towards a fairer, more just and equal society.
    • Ingenuity: We are resourceful and innovative in developing better solution.
    • Determination: We are courageous and ambitious for what we can achieve together.
