Palliative Care in Dementia Liaison Service (part of the wider Mental Health Liaison Service for Older People)
The service provides specialist mental health assessments in a variety of health and social care settings in order to ensure the mental health needs of older people are assessed and supported when they are in the last 12 months of life
The service aims to ensure colleagues in palliative care services are able to access specialist advice and assessment for patients with dementia and for patients with a life limiting condition who may have developed cognitive problems.
The service also provides support and assessment for carers and relatives of patients known to the palliative care service that may have a diagnosed dementia or is experiencing changes in cognitive function.
