Adult Learning Disability Team, Bury
The adult learning disability team in Bury is a multi-disciplinary team made up of Learning Disability Nurses, Occupational Therapy, Speech and Language Therapy and Psychology working together to provide a seamless service for adults from age 18.
We support individuals who have a mild/moderate/severe learning disability who are registered with a Bury GP. We deliver person centred, high quality, health focussed support to enable individuals to develop, maintain or restore a valued life in the community.
This is underpinned by the key principles of Inclusion, Rights, Choice and Independence. The core aim of the service is to deliver a needs led, highly specialist assessment and intervention plan focusing on health inequalities, behaviour, risk, sensory, communication and psychological needs to individuals with complex and challenging health needs.
We will work in partnership with other agencies and / or families to achieve positive outcomes for individuals in the home, community and other relevant settings. We accept referrals from service users, carers, social workers, GP's and other health professionals. All referrals will go through a screening process for eligibility to access the service.
The service was voted by colleagues as winners of the Pennine Care 2015 Principles of Care Awards for their work with people with Down's Syndrome and Dementia.
