Grange View Enhanced Intermediate Care Centre (Tameside and Glossop)
Grange View provides enhanced intermediate care for up to 40 patients who do not need to be in hospital but require more care that can be provided to them at home.
Care is provided by a multi-disciplinary team consisting of nurses, physiotherapists, occupational therapists, GPs, social workers, pharmacy technicians and health care therapy assistants. Care includes assessment, recovery from illness and rehabilitation.
Patients are referred from acute hospitals and the community. They need to be medically fit, be over 18 years and able to participate in the care provided. The typical length of stay in enhanced intermediate care is 2-3 weeks.
On admission, we assess patients and help them set rehabilitation goals to enable them to recover and rehabilitate so that they can return home and live safely.
Care will include re-abling patients to mobilise, dress, manage personal hygiene and prepare simple meals through therapy and group activities.
Patients and carers will be included in decision making, particularly around discharge planning. The team will assess patients and their homes for any equipment needs e.g. mobility aids, rails to enable safe living at home.
Grange View consists of three wards: Jenner, Lister and Blackwell. Each ward has its own lounge area, dining facilities, and single en suite bedrooms. It is located within Hyde Nursing Home.
