We provide a community‑based service for people aged 65 and over with acute mental health needs, and for people under 65 with a diagnosis of dementia who are living in a care home.
We support care homes to manage behaviours that can be challenging.
We work closely with care home staff to help people remain in their current placement and to avoid unnecessary moves to other care settings or hospital admission.
We're a small team made up of a team manager, registered mental health nurses with experience in dementia care, a support worker who works alongside the nurses, and a team administrator.
Support is provided to care home staff through therapeutic approaches, including life story work and advice on developing person‑centred care plans.
We carry out comprehensive mental health assessments in partnership with residents, their families and carers. Where appropriate, referrals can be made to other services, such as psychiatry, memory assessment services and adult social care, and people can be signposted to additional support.
