We're making some planned improvements to the adult community mental health teams in Tameside.

Summary of the changes
Tameside’s mental health services are delivered through a partnership between our organisation and Tameside Council. This means that health and social care professionals work together as one team to meet the needs of our patients.
We've recently reviewed the ways we can improve the service to make sure people have the best possible care and experience.
We believe the best approach is for our organisations to continue working together, but we'll each lead on the areas where we have the most expertise. This will allow us to focus on our strengths but work together when needed.
The plan that we have developed means:
- People with mental health care needs will continue to be supported by mental health professionals working in the existing community mental health teams, provided by Pennine Care.
- People with social care needs due to their mental health will be supported by a new adult social care mental health team, provided by Tameside Council. This means people who find it difficult to manage life due to their mental health, so might be able to get help under the Care Act.
- People who have both health and social care needs will be supported by both teams.
Please don’t worry. We’re absolutely committed to continuing to work closely together to meet your needs. This new approach is designed to make sure everyone gets the right help from the right services.

How might the changes affect you?
Depending on your needs, you may have a different key worker (previously called a care coordinator), which is the person who plans and manages your care.
Please don't worry. If we need to change your key worker, we’ll talk to you, provide you with any support you need and answer your questions.
You and your family and carers will continue to receive high quality care and support that meets your needs.

We're holding our second listening event in late March 2025. This is a chance for you to find out more and ask questions.
You don't need to stay for the full event - you can drop in between the times below and stay for as long as you'd like.
