Community Paediatric Service
We are part of the Children’s Acute and Ongoing Needs Service (CAONS). Click here to find out more about the CAONS.
We support children and young people with complex, or life-limiting conditions. Working closely with the children’s community nurses we aim to provide care closer to home.
We also provide:
- Medical support to special schools in the borough
- Medical assessments for children in foster care
- Medical advice for children applying for an education health care plan
- Medical advice as part of the adoption process
Our team includes community paediatricians, a specialist community doctor, an advanced nurse practitioner, and health care support staff. We are supported by a team of medical secretaries and administration staff.
We want to diagnose neurodevelopmental problems as early as possible, so we can offer children and young people the right support. Our aim is to make sure they can reach your full potential.
How we will help?
We will provide every child with a medical developmental assessment. This will involve carrying out investigations to help us understand their condition and needs. We then then plan the right care for them.
How to refer to us
We are only able to accept referrals from a health, social care or educational professionals. Unfortunately we are not able to accept self-referrals or referrals from parents or carers.
