Community Paediatric Service

The HMR Community Paediatric Service undertakes multi-agency assessments, diagnose and plan the care management for children with neuro-developmental concerns, neuro-disabilities or complex health needs and children with social communication difficulties and behavourial concerns. 

We offer: medical advice for children with special educational needs; statutory health assessments for those about to be adopted and fostered; assessments for children with suspected autistic spectrum disorder and muti agency team assessments. Clinics are mainly held at Croft Shifa Health Centre and the borough's special schools.

Our team consists of paediatricians,  advanced children's nurse practitioner, physiotherapists, occupational therapists and Spech and language Therapists. We liaise closely with the Area SENCo team within the local authority. 

The designated medical officer (DMO) for children with special educational needs (and disabilities) (SEND) is based within the service. The DMO offers a link with the local authority and will advice or assist with any medical assessments for SEN whilst acting as a resource for information regarding children with special education needs. 

Much of the work is multi-disciplinary and multi-agency, and there are numerous interdependencies with other services in health, social care and education.

The Community Paediatric Service is part of the Children’s Acute and Ongoing Needs Service (CAONS), which launched in September 2016.  Click here to find out more about the CAONS.

The CAONS include a number of specialisms for children and young people (details via above link) and all can accessed via a Single Point of Access (contact details below).

The address provided below is a central hub for all specialisms. The CAON team provides services in children's homes and at a range of community-based venues.
