Heywood, Middleton and Rochdale Children’s Diabetes Specialist
The Children's Diabetes Service is for parents and children within HMR diagnosed with diabetes. Newly diagnosed patients are referred direct from the hospital. Other health professionals can refer if any families are movements into the area for ongoing support.
The service provides ongoing community diabetes education and support for children/young people 0-19 years and their families. Aiming to promote awareness and understanding enabling them to lead full healthy active lives and reduce unnecessary hospital attendances.
The children’s diabetes specialist is part of the Children’s Acute and Ongoing Needs Service (CAONS), which launched in September 2016. Click here to find out more about the CAONS
The CAONS include a number of specialisms for children and young people (details via above link) and all can accessed via a Single Point of Access (contact details below).
The address provided below is a central hub for all specialisms, however the children’s diabetes specialist provide services in people’s homes and at a range of community-based venues.
