Heywood, Middleton and Rochdale Children’s Complex/Palliative Care
We offer support to babies, children or young people with a life-limiting or life-threatening condition, along with advice and help for their families.
We aim to provide high quality nursing care and knowledge to make sure each baby, child or young person experiences the best health and quality of life possible throughout their journey.
The child and family are at the centre of the care we provide and we ensure that it meets their individual needs.
We discuss these needs with the family and then offer advanced care planning, care co-ordination, management of symptoms, end of life care and emotional support.
We provide a seamless and co-ordinated service which aims to care for babies, children and young people with complex health needs at home. The aim is to reduce the need for a hospital admission or reduce length of stay.
We offer holistic care (i.e. that meets all the child's various needs). While a significant part of our role is in to identify the right services to support them and co-ordinate this care, when required, we also support the family by providing care packages tailored to meet their needs when required.
We also support young people to trsnsition from children's to adult services.
