Bury and Heywood, Middleton and Rochdale Adult Diabetes Service
The Bury and Heywood, Middleton and Rochdale (HMR) Diabetes Service launched in September 2014.
It has seen community-based diabetes staff join together with diabetes staff from Fairfield General Hospital and Rochdale Infirmary join together form a new single service.
The service is for adults, aged 18 years and above, who have diabetes and are registered with a GP in Bury or the Rochdale borough.
The new service means that the majority of your diabetes care will now be delivered within your local community, at a health centre, clinic or GP practice.
Depending on your needs and the complexity of your condition, you could be cared for by the Bury and HMR Diabetes Service directly, or even by your GP or practice nurse.
Only people needing the most complex and specialist services will now receive their care at Fairfield General Hospital or Rochdale Infirmary.
The service will also provide training and education for GPs and primary care clinicians to support them to effectively manage your condition and reduce the risks of any further complications.
You will also be supported to improve your ability to manage your condition through a nationally accredited programme.
