In Trafford we provide a range of community services that aim to keep you healthy from the day you are born through to the end of your life. We also provide Child and Adolescent Mental Health Services (CAMHS).
Our services are integrated with Trafford Council, which means our health teams work in partnership with the council’s social care teams to make sure your care is joined up. This supports our commitment to providing integrated services that ensure you can benefit from ‘whole person care’.
Most of our teams are structured and managed by ‘neighbourhood’. These neighbourhoods are defined as north, west, south and central Trafford and we work closely with the GP leads for each of those areas which helps to ensure that, where possible, residents receive care closer to home.
Services in Trafford are led by Integrated Network Director, Richard Spearing, with support from Diane Eaton, Director of Integrated Services, and Cathy Rooney, Director of Safeguarding and Professional Development.
In addition to a wide range of nursing and therapy services, in Trafford we also provide urgent care to help support patients at home and prevent them being admitted to hospital - this is delivered by the Community Enhanced Care (CEC) service.
