Community services are best described as a range of nursing and therapy services that are provided outside of hospital, usually at a health centre, GP practice, community venue or in your home.
They aim to keep you healthy and support you to stay out of hospital, from birth right through to the end of life.
We provide community services in:
- Heywood, Middleton and Rochdale
- Trafford (integrated with Trafford Council)
Examples of the community services we provide in these areas include:
Adults: audiology (hearing), cardiac (heart), diabetes, district nursing, occupational health, physiotherapy, podiatry, sexual health and speech and language therapy
Children: audiology, children’s community nursing, dietetics, health visiting, school nursing, speech and language therapy
We also run other specialist community-based services, such as intermediate care facilities, which vary in each borough.
