HMR children's community nurses move

The Heywood, Middleton and Rochdale children’s community nurses (part of the Children’s Acute and Ongoing Needs Service - CAONS) are moving from their current base at Kingsway Clinic.

From 13 February 2017 the children’s community nurses will be based at Callaghan House, Cross Street, Heywood, OL10 2DY.

Clinic changes:

As a result of the move there will be some changes to clinic venues. However, clinics will continue to be offered at various locations across the borough.

There is no change to the referral process for the clinics – which is via the CAONS Single Point of Access (details below).

Following the referral, the CAONS will advise which clinic to attend.

There will also be changes to the collection point for some equipment. Again, the CAONS will advise when and where equipment should be collected.

Referral process:

Referral to the children’s community nurses will continue to be via the CAONS Single Point of Access as follows:

  • Telephone: 01706 676777
  • Fax: 0207 019 7419
  • Email: pcn-tr.HMRChildrensAONS@nhs.net
  • Post: Children's Acute and Ongoing Needs Service, Callaghan House, Cross Street, Heywood, OL10 2DY

Referrals can be made by anyone who works with a child or young person, including health professionals, social care professionals and education professionals.

Benefits:

The CAONS launched in September 2016, bringing together a range of children’s health professionals, including the children’s community nurses, into one service.

The move is part of plans to develop Callaghan House as a central children’s hub. The aim is to ensure local children, young people, families and carers benefit from more joined up care, which will ultimately help to ensure they experience the very best health and wellbeing.
